Novo Nordisk’s Next-Generation Obesity Drug CagriSema Shows 25% Weight Loss in New Women-Focused Analysis

People walk past a sign for Novo Nordisk’s annual general meeting at the entrance to the venue in Copenhagen, Denmark, March 26, 2026.

Danish pharmaceutical giant Novo Nordisk announced on Tuesday that its highly anticipated next-generation obesity treatment, CagriSema, demonstrated a 25% average weight loss among adult women in a newly released subgroup analysis conducted following a pivotal late-stage clinical trial.

While the company emphasized that further research and validation are required to fully confirm the hypotheses generated by these two post-study analyses, the findings provide fresh evidence that the experimental weekly injection can build upon and improve the significant weight reduction and broader health benefits offered by Novo’s established GLP-1 therapy, semaglutide. Semaglutide serves as the foundational active pharmaceutical ingredient in the company’s blockbuster chronic weight-management medication Wegovy and its diabetes counterpart, Ozempic.

CagriSema represents a combination therapy that pairs semaglutide with another innovative compound called cagrilintide, which specifically targets the human body’s amylin pathway. Industry experts widely expect the combination drug to secure regulatory approval and launch as early as next year. In the fiercely competitive landscape of metabolic disease treatments, obesity drug market leaders Novo Nordisk and Eli Lilly have increasingly turned their research focus toward developing new treatments centered around amylin. Amylin is a naturally occurring peptide hormone released by the pancreas alongside insulin that plays a vital role in regulating appetite, hunger signals, and the physical sensation of fullness after eating. Both pharmaceutical titans hope these novel mechanisms will unlock robust new revenue streams that extend well beyond the commercial lifecycle of existing GLP-1 medications.

In the newly revealed analysis, which evaluated approximately 2,300 women diagnosed with obesity or who were classified as overweight, treatment with CagriSema was associated with an average weight reduction of 25% at the 68-week mark. This contrasted sharply with an average weight loss of just 3% observed in participants who received a placebo. It is important to note that these specific figures are derived from an ideal trial scenario in which all participating patients successfully remained on the active treatment protocol without experiencing any major interruptions or discontinuations.

A second detailed analysis examined roughly 1,900 women from the trial by grouping them according to their specific menopausal status. The results demonstrated that weight loss reached 26.7% in premenopausal women, 24.8% in perimenopausal women, and 23.1% in postmenopausal women at the 68-week milestone, assuming all patients maintained continuous adherence to the therapeutic regimen. By comparison, individuals across all menopause groups who were administered the placebo experienced approximately 2% weight loss over the same timeframe.

Furthermore, Novo Nordisk noted that across both of these evaluable analyses, the administration of CagriSema was consistently linked to notable reductions in visceral and abdominal fat, as measured by decreases in waist circumference, both among women overall and across the various stages of menopause.

These granular findings strongly suggest that female patients may experience a marginally greater degree of weight reduction when treated with CagriSema than the broader, mixed demographic population of both men and women with obesity observed in the primary Phase 3 clinical trial.

Initial top-line results from the broader study previously indicated that CagriSema helped adult patients achieve an average weight reduction of 22.7% at 68 weeks, compared to a modest 2.3% improvement in the placebo cohort. However, it is noteworthy that the overwhelming majority of patients enrolled in that trial—approximately 67% of the roughly 3,400 total participants—were women.

The physiological challenge of weight management for women is deeply intertwined with hormonal shifts, as weight gain is exceptionally common during the menopausal transition. As women age and experience declining estrogen levels, their metabolic rate frequently slows down, which can facilitate easier fat accumulation while making the crucial preservation of lean muscle mass significantly more difficult. Once a woman enters postmenopause, estrogen levels generally remain persistently low and do not recover to their previous reproductive-era baselines.

According to data compiled by the U.S. Census Bureau, there are more than 86 million women aged 40 and older living in the United States alone—the precise chronological window during which perimenopause, menopause, and postmenopause typically take place.

Michelle Skinner Moore, Novo Nordisk’s interim head of clinical development, medical and regulatory affairs, underscored the clinical significance of these demographic-specific insights. She stated that many women grappling with overweight or obesity frequently encounter frustrating weight gain as they transition through menopause, making it vital to thoroughly investigate investigational products like CagriSema across the entire spectrum of the female reproductive lifespan.

Despite these encouraging scientific readouts, Wall Street has historically exhibited a somewhat muted reaction toward CagriSema. The drug’s previous late-stage clinical trial results initially disappointed financial market investors because the observed weight loss efficacy figures came in slightly below their lofty expectations. Novo Nordisk management, however, continues to maintain that CagriSema is both clinically effective and sufficiently differentiated from its existing portfolio of medications to command substantial commercial demand upon its eventual market introduction.

Looking ahead to the commercial potential of the therapeutic candidate, equity research analysts surveyed by FactSet project that CagriSema will generate approximately $11.84 billion in worldwide sales specifically dedicated to obesity care by the year 2028.
